On September 26, 2024, Citadel of Northbrook was informed that its electronic health records platform provider, PointClickCare, had experienced a data security breach affecting some of its patient information.
PointClickCare said it discovered the incident on July 20, 2024 and began an investigation. The investigation revealed that an unauthorized actor had accessed and obtained a limited amount of patient information using compromised end-user credentials. Information compromised in the PointClickCare data breach as it relates to patients of Citadel of Northbrook includes names, dates of birth, Social Security numbers, Medicare/Medicaid ID numbers, treatment and diagnosis details, admission and discharge dates, prescription data and health insurance policy numbers.
