CardioVascular Health Clinic, an Oklahoma-based provider of heart and vascular care, reported a data breach to the U.S. Department of Health and Human Services on May 3, 2025. An investigation into the incident revealed that unauthorized access to network information occurred between February 18 and March 4, 2025 due to an incident with the clinic's cloud services provider.
