The types of information potentially accessed vary by individual but may include names, addresses, email addresses, phone numbers, dates of birth, dates and costs of service, medical diagnoses, treatment details, health insurance information, and, in limited cases, Social Security numbers. Individuals affected by the Cardiology Associates of Fredericksburg data breach will receive notification letters (provided their mailing address is available) with details about what personal information may have been involved.
