Azle Smiles, a general and cosmetic dental practice in Texas, has reported a data breach involving sensitive patient information.
According to a notice posted to its website, Azle Smiles was informed by its IT support company of irregularities related to remote access session hosts on May 26, 2026. Following an investigation, the practice determined that data impacted by the incident may have included names, addresses, dates of birth, driver's license numbers, government-issued IDs and medical information.
The Azle Smiles data breach was also reported to the Texas Attorney General's Office on July 28, 2026, which noted that health insurance information may have also been exposed.
