Arrowhead Evaluation Services Security Incident: What Happened?
Arrowhead Evaluation Services, Inc., which provides independent medical examination and medical review services to insurance carriers, employers, third-party administrators and law firms in California, recently experienced a data breach. The company detected unauthorized activity and determined that on or around June 14, 2024, certain files containing personal details may have been accessed or acquired.
According to a notice published on the company's website, the types of information potentially involved in the Arrowhead Evaluation Services data breach include full names, dates of birth, Social Security numbers, driver’s licenses, financial account details, alien registration numbers, diagnosis and treatment information, medical data, medical policy numbers, physician details and insurance information.
Arrowhead Evaluation Services stated on its website that not every type of information was impacted for every affected individual, as the specific data varied by person. On June 2, 2025, the company mailed data breach notices to impacted individuals for whom address information was available.
