Anatomic and Clinical Laboratory Associates, a physician-owned pathology group in Tennessee, has reported a data breach affecting 169,626 individuals.
According to a notice posted to its website, the company became aware of potentially unauthorized activity in its computer network on December 1, 2025. Following an investigation and review of affected data, it determined on April 27, 2026 that the personal information affected by the breach included names, dates of birth, Social Security numbers and taxpayer identification numbers.
The notice disclosed that the Anatomic and Clinical Laboratory Associates breach may have also exposed medical dates of service, provider names, mental and physical conditions, medical treatment or procedure information, diagnoses, clinical information, medical history, patient account numbers and medical records. The degree of personal and protected health information affected varied per individual.
The company has sent mailed notices to individuals affected by the breach, a sample of which is shown below.
